There are many people who usually believe that you do need to take the tooth out and install an artificial tooth if it is broken and left only the root.
This idea is totally wrong.
Factors that affect the possibility of maintaining the root
1 - The length of the root which can be calculated by X ray.
2 – Position of the root (above or below the edge of the bone, or at the level of the bone)
3 - The nature of the gum surrounding the root (healthy or inflamed)
4 – Relationship of the root to the jaw and other neighboring structures
5 – Thinking about the practical usefulness and functional and aesthetic outcomes
In general, most cases can maintain the roots especially if the condition meets with the above criterior. When we started practicing dentistry 40 years ago, we always tried to keep the roots in most cases but there were some difficult cases of the root at the level of bony edge. In such cases, we did not overfed it but we resolved by surgical method which is very simple and painless. That is the removal of part of the bone and gum surrounding the root of the partition in order to make it visible above the bone so that we can build up on the industrial crown.
Clinical case
The same patient who we mentioned in our previous article came to our clinic. We inserted an implant in the upper front tooth area. He is a 67 years old Syrian man and came to our clinic four months ago by holding two artificial crowns of upper central and the broken lateral ones. It is found that central incisor can be preserved while the root of the lateral should be extracted and replaced with implants after taking a radiograph.
We took out the broken post from the bone, expanded the root canal, took impression and sent to the dental laboratory to make a core with post from the casting metal to be as a primary crown.
After the initial metal core with post returned from the lab, we have entered in the root canal and fixed with the appropriate bonding material as appeared in the picture as follows

In the second phase, we took the second impression and sent to the dental laboratory for the formation of the final crown of pure ceramic material, which is called cerconium as it appears.
Result
Finally, the patient was pleased and satisfied with the result.
We can provide the best patient outcome by a simple and easy way.
Moreover, patient can save a lot of money and preserve the tooth root.
